LDAP configuration settings

I need some assistance with setting up LDAP authentication on my Forma installation.
Whenever i enable LDAP settings in my setup and try to login, it loads a blank page.
I have a Win2008 R2 Server DC with AD setup and I would like to know the settings to input in the "Username for LDAP users.." text box.

I think the example illustrates the format your user and domain info need to be saved as...
For example if I have the domain abc.usa.com
My user would be something like $[email protected]
Not exactly sure though as I have not enabled this function
